Membership Pricing for Direct Primary Care Services

 

Registration Fees - Required for All Membership Tiers

$150 per individual OR

$250 per household

*Should you leave the practice at any time or payment not go through for monthly membership, then the registration fee will be required to be paid again and services will not be rendered.

 

Monthly Membership - Tier 1

A minimum 6-month commitment is required.

$100 per individual per month OR

$250 for a family of 3 per month and then $50 per additional family member.

So:

$300 for a family of 4

$350 for a family of 5

*A 5% discount is available to those that pay for 1-year in full*

*Discount does not apply to the registration fee*

 

What’s included?

• 1 sports physical per patient per year
• 1 routine physical per patient per year (this includes routine labs: CBC, CMP, lipid panel, TSH, hemoglobin A1C, urinalysis, PSA if needed, pap smear if needed)
• Unlimited phone calls and portal messaging with Katherine and Lindsey during normal business hours (responses within 3 business days)
• 12 office visit/sick visits per year
• Televisits available at providers discretion
• Urinalysis included, all other labs “a la carte”
• Ear irrigations/ear wax removal
• EKG
• Suture/staple removals
• Urine pregnancy testing

$200 per individual per month OR

$350 for a family of 3 per month and then $75 per additional family member.

So:

$425 for a family of 4

$500 for a family of 5

• Tier 1 services
• 12 additional office visit/sick visit per year
• The following testing/treatment as deemed appropriate by provider:
• Strep
• Flu A/Flu B
• COVID
• RSV
• Steroid injections
• B12 injections
• Quarterly blood work for those with chronic conditions requiring monitoring (CBC, CMP, lipids, TSH, hemoglobin A1C); additional testing extra

 

Monthly Membership - Tier 3

A minimum 6-month commitment is required.

$300 per individual per month OR

$450 for a family of 3 per month and then $100 per additional family member.

So:

$550 for a family of 4

$650 for a family of 5

*A 5% discount is available to those that pay for 1-year in full*

What’s included? 

• Tier 2 services
• Unlimited office visits/sick visits
• 1 Myer’s IV infusion per month OR 1L normal saline fluid bolus
• Providers have the right to refuse to give infusions to those for whom it is not medically appropriate
• “Slim Shots” weekly
• Additional testing/treatment as deemed appropriate by provider:
• Ondansetron IM/IV
• Diphenhydramine IM/IV
• Rocephin IM/IV
• Blood work/labs at physical annually: vitamin D, vitamin B12, hormones
• Aesthetic consult
• Discounted pricing on aesthetic services

“A la carte” Services (if not included in membership tier)

Office/Sick Visits:

• $75 for low complexity visit
• $100 for moderate complexity visit
• $150 for high complexity visit

 

In-Office Testing

• Strep test - $30
• Flu A/Flu B - $45
• COVID - $45
• RSV - $50

 

In-Office Medications

• Depo-Medrol - $40
• Ondansetron - $25
• Diphenhydramine - $25
• Rocephin - $40
• Vitamin B12 - $25
• Nebulizer treatment (albuterol) - $25
• Myers’ IV infusions - $150
• Normal saline 1 L infusion - $125

 

Available Vaccinations:

• TDaP - $100
• Meningitis - $200
• Flu Vaccine (When available/appropriate) - $35
• Flu High Dose for over 65 (When available/appropriate) - $75

 

Additional lab testing pricing available by request as needed

 

FMLA/disability forms/All other forms at provider’s discretion - varies depending on length and complexity of the form

Medical records 

• Faxed/emailed records - free of charge
• Printed records - varies depending on quantity of pages/administrative time required - minimum $25
